不同损伤能量对儿童股骨颈骨折的影响

The effects of different violence on fracture of the femoral neck in children

摘　要：目的探讨不同损伤能量水平对儿童股骨颈骨折的影响.方法回顾分析10年来我院小儿骨科收治的63例股骨颈骨折患儿,按照致伤能量的高低分为高能组48例,低能组15例.结果高能组中43.8%(21例)有合并损伤,而低能组却无一发生;合并损伤发生率的高低依次为骨盆骨折、下肢骨折、内脏挫伤和颅脑损伤;高能组发病年龄分布较为离散(3～14.5岁),而低能组发病年龄相对集中(8～13岁);高能组中包含各型骨折,Ⅱ、Ⅲ型多见,而低能组中仅见Ⅱ、Ⅲ型.结论儿童股骨颈骨折多为高能量损伤,近半数的病例有合并损伤;儿童股骨颈骨折首选治疗方法为牵引复位或闭合复位,治疗方法的选择主要取决于患儿年龄、骨折类型和移位程度等因素,而与损伤能量或者合并损伤无关.
